CT-MRI Form 2 (Version 2020) – PBCT-MRI & FTC (Official Application and Clearance Form): for those who started CT-MRI Fellowship training from January 2019 onwards
CT-MRI Form 2 (Version 2019) – PBCT-MRI & FTC (Official Application and Clearance Form): for those who completed CT-MRI Fellowship training between 2014 and 2019
CT-MRI Form 3 (Version 2019) – PBCT-MRI & FTC (Official Application and Clearance Form): for those who completed CT-MRI Fellowship training between 2007 and 2013, and have not yet taken the Philippine Board of CT-MRI
CT-MRI Form 4 (Version 2019) – PBCT-MRI & FTC (Official Application and Clearance Form): for RETAKERS
Deadline of Submission of CT-MRI Form 1 (Registry Form): not later than one(1) month after the first day of training
Deadline of Submission of CT-MRI Forms 2, 3 and 4 (Clearance Form): before the 3rd Monday of March of each year
*** CT-MRI Form No. 2 (Version 2019 & 2020) – will be given only to those who have duly submitted CTMRI Form 1 (FTC form 1/Registry Form)
